Jonathan Gamss

Jonathan Gamss,

MD

Our Medical Director, Dr. Jonathan Gamss MD specializes in chronic disease management and virtual care for Medicare patients. He works closely with each patient to understand their health needs, create personalized care plans, and provide ongoing support for chronic conditions.

Under Dr. Gamss’ care, you will be supported by his dedicated team of nurse practitioners, nurses, care coordinators, and support specialists. Together, they help ensure your care is well coordinated across doctors, pharmacies, and community services, making it simpler to stay on track and get the support you need.

Trained at the Albert Einstein College of Medicine, Dr Gamss completed his Internal Medicine and Emergency Medicine residencies at Long Island Jewish Medical Center and is board-certified in Internal Medicine and Emergency Medicine. He has been taking care of patients and involved in healthcare administration for over 20 years.

Jenny Jagdatt,

MBA, BSN, RN 

Jenny Jagdatt is an experienced healthcare leader whose expertise in regulatory compliance, clinical oversight, and operations strengthens the quality and reliability of care PodHealth delivers. She ensures that strong systems and clear accountability are in place so patients receive safe, ethical, and evidence-based care.

Jenny is known for elevating clinical performance while fostering a culture grounded in patient-centered values. She has led multidisciplinary teams through complex operational improvements, building scalable processes that enhance efficiency without compromising integrity. By aligning operations with federal, state, and accreditation standards, she protects both patients and providers.

Passionate about developing people as well as processes, Jenny’s leadership creates consistency, trust, and better outcomes for the patients and clients PodHealth serves.

Alexandra Levy

Alexandra Levy,

RN

Alexandra graduated from Adelphi University with a bachelor’s degree in Science of Nursing. Alex has wanted to be a nurse ever since she was 9 years old, when she went to visit her grandfather in the intensive care unit after surgery.

One of her greatest achievements as a nurse was when she was working as a Visiting Nurse and was assigned a biweekly visit to a patient who was bed-bound and virtually nonverbal due to a stroke. The patient had terrible bedsores that needed care. At each visit, Alex was assessing the patient’s status and providing wound care. Over several months, she witnessed these awful wounds heal, and saw sores go from open to close. Seeing the happiness of this family as their loved one healed is a memory Alex will treasure forever.

Alex is an avid exerciser; she tries to go for an early morning run and lift weights 6 days a week. As a former psychiatric nurse, she strongly believes that maintaining your mental health is crucial for one’s overall wellbeing.

Larissa Jerminow,

NP

Larissa Jerominow, is a board-certified Adult Gerontology Nurse Practitioner (MSN, RN-BC) with a Master’s degree in Adult Gerontology Nursing from the City University of New York, College of Staten Island. Trained in nutrition and functional medicine, Larissa brings a unique perspective shaped by her Russian heritage, which gives her a deep appreciation of Eastern wisdom alongside Western advances in natural healthcare.

Having personally experienced biotoxin illness from exposure to a water-damaged building, Larissa is determined to help New Yorkers to upgrade their immune systems and brain function for maximum energy, performance, and longevity. She strives to improve access and speed of recovery for patients struggling with chronic and “mystery illnesses”. She goes beyond biological medicine, drawing from all systems and healing modalities-from energetic therapies to nutrition-to help patients reclaim their wellness and reveal their highest potential.

Zeyan Lin,

NP

Zeyan Lin chose to become a Nurse Practitioner because she has a strong desire to help her clients and make a meaningful difference in their lives. She is driven by compassion, patient advocacy, and the opportunity to provide high-quality care that supports individuals and families through their most vulnerable moments.

Being an NP allows Zeyan to combine clinical knowledge with empathy to improve health outcomes and build trusting relationships.

Zeyan has a Bachelor’s degree from Hunter College, and a MS, Primary Care Family Practitioner from St. John Fisher University.

Terri Parkin, Nurses Practitioner

Terri Parkin has been a Nurse Practitioner for 16 years. She is board certified through the American Academy of Nurse Practitioners (AANP) and a member of Sigma Theta Tau International Honor Society, Zeta Iota Chapter.

Terri is certified in Family Practice, with experience collaborating across specialties including Endocrinology, Psychiatry, Cardiology, Gastroenterology, Neurology, Pulmonology, and Orthopedics. She believes in a patient-centered medical model and encourages patients to ask questions and take an active role in their care. Her goal is to give every patient a medical experience built on respect, honesty, and compassion, so they walk away with clear knowledge of their health and practical steps they can act on going forward.

Leticia Mora,

FNP-C

Leticia Mora, FNP-C is a dedicated Family Nurse Practitioner specializing in chronic care management and preventive medicine. A graduate of South University, she is committed to delivering comprehensive, patient-centered primary care that emphasizes prevention, education, and coordinated services to support long-term health and well-being.

Leticia enjoys caring for patients of all ages, providing compassionate guidance and evidence-based care to address a wide range of health concerns. She believes that optimal health is achieved by treating the whole person—not just the condition. By incorporating each patient’s lifestyle, nutrition, personal goals, and individual needs into every care plan, she creates personalized treatment strategies that empower patients to take an active role in their health.

Meet Vanessa Okafor, MSN, AGNP-C, a Nurse Practitioner with over ten years of experience dedicated to providing compassionate nursing care. Vanessa’s holistic care approach is simple: she listens. She believes in understanding the whole person—your lifestyle, concerns, and goals—to provide patient-centered care that is both effective and empowering.

With a strong focus on Primary and Preventive Care, Vanessa can help manage a myriad of care needs. Vanessa brings a wealth of knowledge from diverse clinical settings. Her expertise includes everything from blood pressure and insulin management to dermatology. By using clear patient education and seamless care coordination, Vanessa helps you get better results—with the knowledge and support you need.

Abby Lu is a dedicated Adult-Gerontology Nurse Practitioner with a strong clinical foundation in chronic care management and cardiology. A graduate of NYU Rory Meyers College of Nursing, her practice centers on evidence-based care, patient empowerment, and improving long-term outcomes for diverse and medically complex populations.

Her clinical experience has strengthened her commitment to health equity, ensuring that every patient—regardless of background, identity, or socioeconomic status—receives compassionate, accessible, and high-quality care. She is especially passionate about addressing structural barriers to care and advancing initiatives that promote fairness and inclusion across healthcare systems.

With a deep interest in quality improvement, she applies data-driven approaches to streamline workflows, enhance patient safety, and elevate the overall care experience. She thrives in collaborative, interdisciplinary environments and is motivated by opportunities to innovate, advocate, and contribute to more equitable models of care. Jeniffer Vizoso is a Family Nurse Practitioner at Pod Health, specializing in chronic disease management and virtual care for Medicare patients. Through Pod Health’s Providers on Demand model, she partners with patients to understand their needs, create individualized care plans, and support them in achieving meaningful health goals.

Working closely with a multidisciplinary team of nurses, care coordinators, and social support specialists, Jeniffer helps patients manage medications, attend appointments, and access essential services—from physical therapy to mental health resources. She uses remote patient monitoring to track health trends in real time, enabling early intervention and improved outcomes.

Jeniffer earned her Master’s in Family Medicine from Florida International University and provides virtual consultations focused on personalized, culturally sensitive care and long-term wellness.

Christina Casper | Registered Dietitian | Pod Health

Christina Casper,

Ms. Christina Casper is a registered dietitian. She practices at Pod Health, serving individuals in Brooklyn and the Bronx, NY. She earned her bachelor’s degree in food and nutrition from San Diego State University. She is also certified as both a registered dietitian (RD) and registered dietitian nutritionist (RDN) by the Commission on Dietetic Registration.

Casper focuses on practical, sustainable nutrition strategies that fit into everyday life. She works with people to break down complex nutrition information into clear, realistic steps, helping them better understand food choices, eating patterns, and how nutrition connects to overall well-being and long-term health goals.

Ivette Astudilo | Registered Dietitian | Pod Health

Ivette Astudillo,

Ivette Astudillo is a Registered Dietitian Nutritionist (RDN) at Pod Health where she provides evidence-based and individualized nutrition counseling to patients with multiple chronic conditions. She is certified as a Registered Dietitian in New York State and as a Registered Dietitian Nutritionist by the Commission on Dietetic Registration. She earned her bachelor’s degree in food science and a master’s degree in nutrition from Hunter College.

Ivette is dedicated to helping patients improve their health through personalized, culturally sensitive nutrition care. She is committed to making nutrition information accessible and empowering, ensuring that Spanish-speaking communities receive care in their preferred language. She strives to help patients feel confident and motivated to make lasting dietary and lifestyle changes that support their well-being.

Syreeta Love,

Syreeta Love LCSW serves as Director of Social Work with a strong commitment to delivering compassionate, client-centered care grounded in a psychodynamic approach. With extensive experience across clinical and administrative settings, she specializes in understanding the deeper emotional and relational patterns that influence individual behavior, using insight-oriented practice to support meaningful and lasting change.

Drawing on principles rooted in psychodynamic theory, Syreeta emphasizes the importance of early life experiences, unconscious processes, and interpersonal relationships in shaping mental health outcomes. This perspective informs both direct clinical supervision and program development, fostering an environment where reflective practice, emotional attunement, and therapeutic alliance are central.

Syreeta brings a passion for advancing trauma-informed and culturally responsive services. Through a balance of strategic vision and clinical depth, she works to strengthen systems of care and improve outcomes for diverse populations.

Dr Mohammed Yousuf

Mohammed Yousuf,

Dr. Mohammed Yousuf is a foot and ankle specialist at Pod Health. He received his Doctor of Podiatric Medicine degree from Temple University School of Podiatric Medicine before completing a four-year surgical residency at New York-Presbyterian/Weill Cornell Medical Center. His advanced training in both forefoot and rearfoot surgery has equipped him with extensive expertise in the diagnosis and treatment of complex foot and ankle conditions.

Dr. Yousuf specializes in diabetic limb preservation, wound care, sports medicine, dermatologic disorders of the foot and ankle, routine podiatric care, and traumatic injuries. He combines advanced surgical and clinical expertise with a patient-centered approach, partnering with patients to develop personalized treatment plans that align with their needs and goals. He is committed to delivering exceptional, compassionate care and helping patients maintain mobility, comfort, and quality of life.

Aimee Rains,

PT, DPT

Aimee is a Doctor of Physical Therapy with a strong background in outpatient orthopedics and virtual care. She earned her Doctorate in Physical Therapy from the University of St. Augustine for Health Sciences in Florida and has extensive experience treating a variety of orthopedic, musculoskeletal, balance, and mobility conditions.
Aimee has a strong focus on improving strength, balance, mobility, and overall function. She takes a practical, individualized approach to care, using her clinical expertise to help patients move more confidently, reach their goals, and maintain their independence.
Aimee enjoys building relationships with her patients and helping them get back to doing the things that matter most to them. Outside of work, she loves spending time with her husband and two young children, staying active, and spending time with family and friends.
