How chronic illness affects emotional health
Living with a long-term health condition affects not only the body but also significantly impacts mood, motivation, and mental well-being. At Pod Health, we recognize that physical and emotional health are closely linked. Our Behavioral Health Integration program is specifically designed to support patients with chronic conditions such as diabetes, Chronic Obstructive Pulmonary Disease(COPD), asthma, and heart disease.

Diabetes & emotional health
Managing blood sugar levels, medications, and dietary restrictions every day can be exhausting and overwhelming. People living with type 1 or type 2 diabetes are two to three times more likely to experience depression than those without the condition.
COPD & emotional health
Shortness of breath, reduced physical activity, and dependence on medications or oxygen can make daily life feel isolating and frightening. COPD is one of the conditions most strongly linked to anxiety and depression, yet these mental health effects are often undertreated.
Asthma & emotional health
The unpredictability of asthma, not knowing when the next attack might occur, can create ongoing stress and anxiety. For many patients, especially older adults, worrying about triggers and activity limitations can lead to withdrawal from social life and increased feelings of helplessness.
Heart disease & emotional health
A heart attack, cardiac event, or diagnosis of chronic heart failure can be deeply traumatic. Fear of another event, limitations on physical activity, and the weight of lifestyle changes often lead to depression and anxiety, which, if untreated, can worsen heart health outcomes.
You don't have to manage this alone. Our Behavioral Health Integration program is available to qualifying Medicare members who are managing chronic conditions. If you're feeling overwhelmed, anxious, or down, we will listen and help.
As a Pod Health patient, you may qualify if:
You don't need a formal diagnosis to get support. Our BHI program is open to anyone who could use a little extra care.
Qualifying conditions
- You have a current or past history of depression, anxiety, ptsd, or bipolar disorder
- You are feeling lonely, withdrawn, unmotivated, or unhappy, even if you've never been formally diagnosed
- You screen positive for low mood or stress during your intake process with Pod Health or a routine telehealth visit
- Your regular doctor may ask us to call if they notice you seem sadder or more isolated than usual
- You are managing a chronic condition such as diabetes, COPD, asthma, or heart disease

How does virtual
behavioral health care work?
Enrollment through telehealth or Remote Patient Monitoring
Many people join the program after signing up for our Telehealth or Remote Patient Monitoring (RPM) services for blood pressure or glucose tracking. During that appointment, we also screen for emotional health.
Our team reaches out to you
Our team will reach out to offer you enrollment in our Behavioral Health Integration program, which is covered by Medicare for qualifying patients. You can join at any time, when you feel comfortable.
Monthly check-in calls
Once you're enrolled, someone from our team will call you once a month to see how you're feeling. These calls are pressure-free; you can simply talk, and we're here to listen.
Support anytime you need us
If something changes in your health or care, we'll check in to make sure you're doing okay. And if you feel the need to speak with us before your next virtual scheduled visit, you can reach out to us directly.
