Managing diabetes
is easier with a team
Pod Health supports Medicare members living with diabetes through remote monitoring, monthly care calls, behavioral health support, and nutrition counseling — all from home, at no out-of-pocket cost for eligible members.
Free eligibility check · No commitment · Takes 5 minutes

Medicare covers all four services for eligible members, with most patients paying $0 out of pocket. Copays and deductibles may apply. Pod Health works alongside your existing doctors — not instead of them.
What Pod offers for diabetes
Remote Patient Monitoring
At-home devices track blood glucose, blood pressure, and weight daily. Your clinical team reviews every reading and contacts you if anything is out of range.
Monthly care coordination calls
Stay on track between visits with a monthly call from your care coordinator — reviewing your readings, medications, and care plan.
Behavioral Health Support
Regular emotional wellness check-ins to support mood, motivation, and coping with the daily demands of living with a chronic condition.
Nutrition counseling
One-on-one sessions with a Registered Dietitian focused on meal planning, blood sugar management, and building sustainable eating habits.

What our patients are saying
I didn’t even know I was missing benefits until Pod called. They helped me set up home care and got my Medicaid paperwork sorted. I didn’t have to do a thing. They really looked out for me.

I was trying to help my mom get care, but it was so confusing. Pod walked me through everything in plain English and followed up until she had services at home. I honestly don’t know what we would’ve done without them.

Hablaron mi idioma, me ayudaron con los formularios y me llamaron cuando dijeron que lo harían. Eso importa.
