Hypertension (High Blood Pressure)

Blood pressure readings at home are often more accurate than in a clinical setting. We monitor your daily readings, spot trends, and reach out if something needs attention, without you having to do anything except use your cuff.

Diabetes (Type 1 & Type 2)

Your glucometer sends readings directly to our team. We track patterns over time, coordinate with your doctor when needed, and help you understand what the numbers mean — in plain language, not medical jargon.

Chronic Heart Disease

Daily weight is one of the most important early signals in heart failure management. A sudden increase can mean fluid is building up around the lungs. Our scale catches that fast, often before you feel symptoms.

COPD, Asthma & Respiratory Conditions

Your pulse oximeter tracks blood oxygen levels every day. If your oxygen drops, we see it. For patients managing COPD or asthma, that early warning matters.

Chronic Kidney Disease

Blood pressure and weight monitoring are central to managing CKD. We coordinate with your nephrologist and flag anything that could indicate your kidneys are under additional stress.

Obesity & Metabolic Conditions

Daily weight tracking, combined with regular check-ins from your Pod Health care team, creates accountability and an early warning system, without feeling like you're managing it alone.
