What is Remote Patient Monitoring?

Remote patient monitoring, or RPM, is a form of virtual care that uses connected devices, like a blood pressure monitor or glucometer, to track your health data at home. Your readings are sent automatically to your care team, who reviews them and follows up if anything needs attention. No extra effort needed on your end.

How does Remote Patient Monitoring work?

Once your remote patient monitoring device is set up, taking a reading becomes part of your normal routine. Your reading is transmitted automatically and reviewed by your care team in the background.

That matters especially in Florida, where the climate brings its own health swings. Year-round heat and humidity can push blood pressure and blood sugar out of range, particularly for members with heart disease, diabetes, or COPD. If a reading falls outside a healthy range, your care coordinator reaches out before you notice anything is wrong.

Supporting Florida's Medicare community

Florida is home to one of the largest Medicare populations in the country, including many retirees managing multiple chronic conditions. POD Health’s team understands the realities of coordinating care across snowbird schedules, seasonal address changes, and Florida’s mix of urban and rural healthcare access. Year-round or seasonal, your devices keep your readings flowing, with in-person support at our Tampa location.

How POD Health supports Remote Patient Monitoring

POD Health brings together devices, monitoring, and real follow-up in one coordinated remote patient monitoring program, covered by Medicare for eligible members.

At-home monitoring devices

We ship you the connected devices you need to track your blood pressure, blood sugar, weight, and more, based on your specific condition. Devices arrive ready to use, with setup support whenever you need it.

Personalized monitoring goals

Your monitoring plan is built around your specific conditions, not a one-size-fits-all checklist, so you are only tracking what actually matters for your health.

Automatic data streaming

Your readings are sent automatically through a connected cellular device. Our care team reviews your numbers according to your care plan.

Dedicated care coordinator follow-up

If a reading falls outside a healthy range, our care coordinator reaches out to you directly, one person who knows your health history.

Monthly care coordination calls

Our care coordinator calls you every month to review your readings, check in on your monitoring plan, and adjust it as your needs change.

Telehealth appointments

POD Health connects you with your care team by phone or video without leaving home, so you have clinical support between your regular doctor visits.

Coordinated with your doctor

Our care team shares your monitoring data with your existing doctors, so your readings stay part of your full care picture, not siloed information only you can see.

Ongoing health monitoring

Your readings are tracked over time, so nothing has to wait until your next appointment to be caught.

In-office visit

You are welcome to come in for an office visit to meet our care team, get your monitoring devices set up in person, and ask any questions before you get started. Visit us in Tampa. Find our locations here.

What our patients are saying

I didn’t even know I was missing benefits until Pod called. They helped me get set up with the right support and got my paperwork sorted. I didn’t have to do a thing. They really looked out for me.

Angela T., (Age 72)

Brooklyn

I was trying to help my mom get care, but it was so confusing. Pod walked me through everything in plain English and followed up until she had services at home. I honestly don’t know what we would’ve done without them.

Hablaron mi idioma, me ayudaron con los formularios y me llamaron cuando dijeron que lo harían. Eso importa.
