Your cardiac CT came back, and the report mentions two things: a calcium score and something called soft plaque. Your cardiologist discussed both in the same visit. They sounded related. But when the conversation shifted to soft plaque, something in the room changed.
They are related. Both are forms of buildup inside your coronary arteries. But they behave differently, carry different risks, and change the treatment conversation in ways that are worth understanding before your next appointment.
What Is a Calcium Score? What It Actually Measures
A coronary artery calcium (CAC) score measures the amount of calcified, hardened plaque in the walls of your coronary arteries. It does not measure soft plaque. It does not measure total blockage. It is specifically a count of calcified deposits, expressed as a number called an Agatston score.
Calcified plaque is old plaque. It is the end stage of a long, slow process in which a lipid-rich deposit inside the artery wall has hardened into scar tissue over years or decades. It is stable, meaning it does not rupture. But its presence tells you that atherosclerosis has been underway for some time.
Score of 0: Is This a Normal Calcium Score?
No detectable calcified plaque. Very low near-term risk of a cardiac event. A score of zero does not rule out soft plaque, which a calcium score cannot detect, but it is reassuring. Most cardiologists manage a score of zero with lifestyle guidance and monitoring only, unless other risk factors are present.
Score of 1 to 99
Mild calcified plaque. Low to moderate risk. The conversation at this level typically involves lifestyle modification and a discussion about whether to start a statin. Many physicians use a calcium score in this range, along with a 10-year cardiovascular risk calculator, to make decisions. Annual monitoring and periodic repeat imaging are common.
Score of 100 to 299
Moderate calcified plaque. Risk is meaningfully elevated. Most cardiologists will recommend statin therapy at this level regardless of other risk factors. The goal shifts from prevention to active management: lowering LDL, controlling blood pressure, and reducing inflammation. Stress testing may be ordered to evaluate whether any blockages are affecting blood flow.
Score of 300 or Above: A Dangerously High Calcium Score
Extensive calcified plaque. High cardiovascular risk. High-intensity statin therapy is standard, additional medications are often added, and closer monitoring is typical. Scores above 400 are associated with a significantly elevated risk of cardiac events over the following decade.
Percentile ranking
Coronary artery calcium scores are also reported as a percentile relative to people of the same age and sex. A score at the 75th percentile means more calcified plaque than 75 percent of comparable people, even if the absolute number seems modest. Percentile rank often tells a more useful story than the raw number alone, particularly for younger patients whose scores may be low in absolute terms but high for their age group.
What Is the Treatment for Calcified Plaque in Arteries?
With calcified plaque and no soft plaque findings, the clinical conversation is generally one of measured, long-term management.
Diet is recommended across all score bands above zero, with the intensity of the conversation scaling with the score. The Mediterranean diet has the strongest evidence base. For someone with a calcium score in the mild-to-moderate range, dietary changes are framed as a long-term strategy for cardiovascular risk reduction rather than an acute intervention.
Statins become increasingly standard as scores rise. At lower scores, the decision may be individualized based on overall risk profile. By the time a score reaches 100 or above, most guidelines support statin therapy. The primary goals are lowering LDL and slowing further calcification, not stabilizing existing plaque, which is already stable by definition.
Stents or other procedures are generally not discussed based on a calcium score alone. A calcified blockage at 50 to 69 percent that is not causing symptoms and not demonstrably restricting blood flow would typically be managed medically, not mechanically. Many patients with calcified blockages in this range are managed medically for years.
Monitoring at moderate calcium scores typically involves repeat imaging in three to five years, annual cardiology visits, and stress testing if symptoms develop.
What Makes Soft Plaque More Dangerous? Stable vs. Unstable Plaque Explained
Soft plaque is a fundamentally different finding, and a calcium score of zero does not rule it out.
Unlike calcified plaque, soft plaque is composed primarily of a lipid-rich core of fat-engorged immune cells, covered by a thin, fragile fibrous cap. It appears on cardiac CT as low-density or mixed-density material rather than the bright white of calcification. It can be present even when the calcium score is zero, which is one reason cardiac CT angiography (which uses contrast dye and captures more detail than a calcium score alone) provides a more complete picture.
The critical difference is rupture risk. Calcified plaque is stable plaque: the fibrous cap has essentially been replaced by hard scar tissue. Soft plaque, by contrast, is unstable plaque, meaning it can rupture. When the thin cap over the lipid core tears, the contents spill into the bloodstream and trigger rapid clot formation. If that clot blocks the artery, the result is a heart attack. This mechanism, plaque rupture followed by acute thrombosis, accounts for the majority of heart attacks, and it can occur at blockage levels that would otherwise seem moderate.
This is why a soft plaque blockage of 50 percent can be more dangerous than a calcified blockage of 70 percent.
How Blockage Percentage Interacts with Plaque Type
The percentage of arterial blockage, called stenosis, is reported as a range on cardiac CT because the measurement is inherently variable.
Less than 25 percent is minimal stenosis, usually managed with lifestyle modification only. From 25 to 49 percent, the approach is medical management and monitoring. At 50 to 69 percent, plaque type becomes particularly important: calcified plaque at this level is often watched, while soft plaque at this level is treated more urgently. From 70 to 99 percent, procedural intervention is more likely to be on the table regardless of plaque type, particularly if symptoms are present or blood flow is objectively restricted. A total occlusion means the artery is completely closed, and collateral circulation and timing determine the clinical approach.
For calcified plaque, the percentage of blockage drives most treatment decisions. For soft plaque, the composition can override the percentage. A 50 percent soft plaque finding may be treated as aggressively as a 70 percent calcified one, because rupture risk is the primary concern, not flow restriction.
Calcified Plaque vs. Soft Plaque: The Treatment Difference in Practice
The same calcium score of 99 at the 75th percentile can lead to two very different clinical conversations depending on what else the scan shows.
With calcified plaque only at that score, the conversation is likely: start a moderate to high-intensity statin, adopt a heart-healthy diet, schedule a follow-up in six to twelve months, and call if symptoms develop. The tone is managed concern, real enough to act on, stable enough not to rush.
With soft plaque present, particularly in a major vessel like the left anterior descending artery, the conversation shifts. High-intensity statin therapy moves to the front, because statins stabilize the fibrous cap directly in addition to lowering LDL. Stress testing or further imaging is typically ordered promptly to evaluate blood flow. Procedural intervention becomes a live question rather than a distant one.
Diet remains relevant in both scenarios. In the soft plaque context, it works as an active tool for reducing the lipid core and dampening the inflammation that keeps the plaque unstable, not just a long-term habit for general wellness.
What to Do If Your Cardiac CT Scan Results Show Either Finding
Both calcified and soft plaque findings warrant follow-up with a cardiologist. Neither should be left without a management plan.
A few questions worth raising at that appointment, regardless of which finding applies:
- Ask what your LDL particle size is, not just your total LDL number.
- Ask whether the blockage is affecting blood flow, and how they will determine that.
- Get a specific LDL target and understand what medications will get you there.
- Find out whether working with a dietitian alongside medication makes sense for your situation.
- Ask which symptoms should prompt you to call before your next scheduled visit.
